NETECH Rural Health Catalyst Fund Privacy Policy

Invest Nebraska administers the NETECH Rural Health Catalyst Fund under a subaward from the Nebraska Department of Health and Human Services (“DHHS”). DHHS is the prime recipient of Nebraska’s Rural Health Transformation Program award from the Centers for Medicare & Medicaid Services (“CMS”).

When you submit information through the Site, through the Cohort 1 application, or by contacting us directly, we may collect and process that information as described in this Privacy Policy.

Certain information submitted to or maintained by Invest Nebraska in connection with the program may also be subject to applicable public records laws.

• Contact information. This may include your name, organization, email address, phone number, and other contact details you provide.

This may include legal name, address, year founded, state of incorporation, entity type, EIN, website, employee counts, and information about your organization, product, market, team, regulatory status, intellectual property, rural health impact, outcomes, and sustainability.
• Financial information. This may include prior funding raised, revenue, net income, financial projections, requested award amounts, proposed budgets, and milestones.
• Documents you submit. This may include a Budget & Milestone Workbook, pitch deck, financial statements and projections, capitalization table, and clinical, outcomes, or other supporting materials.
• Information submitted through contact forms. This may include your name, organization, email address, phone number if provided, topic, message, and the legal applicant name if your question is about a submitted application.
• Usage data. Our service providers may automatically collect technical information associated with use of the Site, such as IP address, browser type, pages requested, and the date and time of a request.

How We Collect Information

Whether we collect certain types of information depends on how you use the Site. We may collect information in the following ways:

• From you. We collect information when you submit an application, upload documents, contact us, submit a question, or otherwise voluntarily provide information through the Site.
• Automatically through the Site. Our hosting and technology providers may collect standard technical information necessary to operate, secure, and maintain the Site.
• From our service providers. Third parties that help us operate the Site or administer the program may process information on our behalf.

How We Use Information About You

We use information collected through the Site to administer the NETECH Rural Health Catalyst Fund and provide the services associated with the program.

• With program administrators and reviewers. Information may be reviewed by Invest Nebraska personnel working on the program, the NETECH Advisory Board, Nebraska DHHS, CMS, and other authorized program reviewers as appropriate.
• With service providers performing services on our behalf. We may provide information to vendors that help operate the Site, collect applications, maintain records, communicate with applicants, or otherwise support program administration. These may include Fillout, Cloudflare, Microsoft 365, and Gather.
• When required by law. We may disclose information when required to respond to a subpoena, court order, governmental request, public records request, or other applicable legal or regulatory requirement.
• When you authorize us to do so. We may share information with another party when you request or consent to the disclosure.

Public Records

Information submitted in connection with a program administered under a state award may be subject to the Nebraska Public Records Statutes.

Nebraska law includes protections for certain proprietary, commercial, and confidential information. If your application contains information that you consider confidential or commercially sensitive, you should clearly identify that information as confidential when submitting it.

Invest Nebraska cannot guarantee confidentiality where disclosure is required by law.

Do Not Submit Protected Health Information

Do not submit protected health information or patient-identifiable information through the Site or as part of an application.

Applicants may provide aggregated or appropriately de-identified clinical and outcomes information.

If materials containing patient-identifiable information are submitted, Invest Nebraska may require the information to be removed or replaced before the application can continue through the review process.

Information Storage and Security

We retain information for as long as reasonably necessary to administer the program and comply with applicable legal, contractual, reporting, and records-retention requirements.

Records associated with the federal award may be subject to federal and state retention requirements, including the record-retention requirements applicable to the Rural Health Transformation Program and Invest Nebraska’s subaward.

We maintain administrative and technical safeguards designed to protect information from unauthorized access, disclosure, alteration, destruction, or loss.

Information transmitted through the Site is encrypted in transit where supported. Our service providers maintain their own security practices and safeguards.

No system of electronic storage or transmission can be guaranteed to be completely secure.

Cookies and Tracking

The Site does not use advertising cookies or third-party advertising trackers.

Certain service providers, including Fillout and Cloudflare, may use cookies or similar technologies that are necessary to operate forms, secure the Site, prevent abuse, or provide other essential functionality.

We do not currently use non-essential cookies for targeted advertising.

• Request that inaccurate information be corrected
• Update information associated with an application
• Withdraw an application before an award decision is made, subject to any records we are required to retain by law or program requirements
